Statement of Good Faith:
Include a statement affirming that you have a "good faith belief that the use of the material in the manner complained of is not authorized by the copyright owner, its agents, or the law." -
Verification of Authority:
Provide documentation verifying your authority to act on behalf of the copyright owner. This must include:
a. A copy of your valid government-issued identification.
b. A notarized power of attorney confirming your authority to represent the copyright owner in this matter. -
Accuracy and Signature:
Include a statement affirming that the information in the notice is accurate, and sign the notice under penalty of perjury.
